I think my eye doctor submitted to insurance for extra an extra service they didn't complete I went to the eye doctor last week for a routine exam at an in-network location. During my visit, two different people asked me if I wore contacts, and I told them no, I never have. However, I just got my EOB from my insurance company, and it lists two codes:
