Do you ask people if they speak English, or do you immediately start speaking? A friend and I are travelling around Europe. If we ever need to speak to someone, she immediately starts speaking in English. I've always assumed that was considered rude, and to at least try to ask in their native tongue if they speak english first. Her point was that "if they do they do, if not, they'll just say it.
