I received a distribution statement from Fidelity concerning an old 401k even though it has been two years since I left the job and rolled over the funds to be transferred elsewhere The statement calls it a deminimus distribution and says I had a beginning balance of around $2500 that was subsequently withdrawn and sent to me as a check (Which I have not received, maybe mailed separately??).
