Why are we supposed to file for refunds in situations where the airline clearly has to refund? Shouldn’t it be automatically initiated by them?

Why are we supposed to file for refunds in situations where the airline clearly has to refund? Shouldn't it be automatically initiated by them? Like I'm not talking about weather related issues or even about how different countries have different policies and amounts for refunds. In cases where they've to refund you, why is it not automatically initiated? Why should we go and file and get on phone with them? 🫤 Now I've never faced this situation thankfully, but I always wonder about it ...
